Introduction: Superior cerebellar artery aneurysms are rare lesions. This location comprises 0.2 % of all intracranial aneurysms, around 60 % debut with subarachnoid hemorrhage, frecuently from small aneurysms.
Case report: A 54- year- old famale patient who began with a sudden headache and loss of consciousness that caused her to fall to the ground. She is diagnosed with spontaneous subarachnoid hemorrhage. Angiography was performed, which showed an aneurysm of the left superior cerebellar artery.
Conclusion: The diagnosis of subarachnoid hemorrhage secondary to an aneurysm of the anterior pontomesencephalic segment of the superior cerebellar artery is an unusual finding
